Extruder is leaking out of the top and bottom

Monoprice Select Mini Manual
by mrpenguin7I got the v2 for xmas and I'm loving it so far (my first 3d printer). However, every time I do a print I have to pause halfway through to clear off all the PLA that's leaking out of the sides of the nozzle and the top part.
I tried cleaning everything and I think I did okay (there was still a little PLA on the nozzle threads, not sure how to get this off). However, even after tightening both the heatsink and the nozzle as much as I could without feeling like I was breaking it, it still leaks. Furthermore, the nozzle actually unscrews a little during the print. Every hour or so I have to pause the print, peel off the PLA with tweezers, and tighten the nozzle again.
I'm not sure what to do at this point. Do I just buy a new nozzle? Is there a better way to clean off my existing one? Am I doing something wrong?
